Campus Assessment, Referral, and Evaluation

The Upstate CARE (Campus Assessment, Referral, and Evaluation) Team supports the safety, health, and well-being of all students, faculty and staff. Its goal is to provide a structured, 解决可能对社区和其他学生产生不利影响的学生行为的积极方法, or may involve mental health and safety issues.

Submit an “R U OK?” / Student CARE Team Referral

任何认为学生对自己或社区构成风险的人都可以向CARE Team转介. When an “R U OK?” or Student Affairs Reporting Form is received, 援外关怀小组评估报告并制定回应. 回应可能包括转介到支持学生福祉和安全的大学资源.

When to register a CARE Team Referral

    • Illegal drug use
    • Disordered eating
    • Self-harming behaviors

Signs or Signals to Consider Before a CARE Team Referral

    • Significant changes in energy
    • Frequent state of intoxication
    • Significant weight loss
    • Worrisome changes in hygiene
    • Noticeable cuts, bruises and burns
    • Sleeping in class or at other inappropriate time
    • Inappropriate emotional outbursts
    • Direct statements indicating distress
    • Expressions of hopelessness or suicide
    • Peer concerns for another student

Reviewing the CARE Team Referral and Initial Meeting

当护理团队收到转诊时,可能会采取一些回应. After a review of concerns in the Care Team referral, 团队成员可以给学生打电话登记和/或给她/他发送可能对她/他有帮助的资源信息.

或者,团队可以邀请一名学生参加会议,一对一地讨论所关注的问题. 会议的目的是确定一个行动计划向前推进,将促进学生的福祉和成功在上州. In some instances, 关怀小组可能会要求学生参加咨询服务的评估, including Psychiatry Services.

  • 评估过程通常由1-2小时的会议组成.  Counseling Services, including Psychiatry Services, 评估后会向学生和护理小组提供治疗建议吗.

  • 在收到咨询和精神病学的建议后,CARE团队可能会安排一次病例管理会议来讨论治疗建议, 协助学生与治疗提供者预约, and answer any questions the student may have.

    如果学生被转介到校外治疗机构, 学生行为办公室的个案经理/协调员将帮助学生寻找提供者,并确保向关怀团队提供适当的文件.  第一次在校外治疗机构预约, 学生获取保险信息,并期望完成初步文书工作和评估.

  • 根据1974年《澳博app下载网》(FERPA)和1996年《澳博app下载网》(HIPAA), 对学生行为和健康记录的访问被严格限制在每个报告所需的最低数量的CARE团队成员, incident, or issue. Before any user is granted access to student records, 用户将承认保密/学生隐私规定. On an annual basis, 风险管理总监将审查用户名单,以确保只有必要的个人才能访问学生记录, 他们也承认了保密条款. CARE Team cases are student-centered, not punitive, 并且不会出现在学生的学习记录或成绩单上. 没有学生明确的书面许可,病例记录是保密和/或私人的, unless there is an immediate health or safety risk.
