Campus Assessment, Referral, and Evaluation
The Upstate CARE (Campus Assessment, Referral, and Evaluation) Team supports the safety, health, and well-being of all students, faculty and staff. Its goal is to provide a structured, 解决可能对社区和其他学生产生不利影响的学生行为的积极方法, or may involve mental health and safety issues.

Submit an “R U OK?” / Student CARE Team Referral

Reviewing the CARE Team Referral and Initial Meeting

根据1974年《澳博app下载网》(FERPA)和1996年《澳博app下载网》(HIPAA), 对学生行为和健康记录的访问被严格限制在每个报告所需的最低数量的CARE团队成员, incident, or issue. Before any user is granted access to student records, 用户将承认保密/学生隐私规定. On an annual basis, 风险管理总监将审查用户名单，以确保只有必要的个人才能访问学生记录, 他们也承认了保密条款.
